Registration Form
Print form and return with
payment. Front & back of form must be signed.
Returning players must submit
player book with registrations.
New players must include a
photocopy of valid ID (driver's license,
passport etc.)
A digital player picture is required. Camera available at registartion or provide by emai or CD.
New form to
be used.
Old forms will no longer be accepted by Peel Halton Soccer Association.
